1. The Dura Mater and its Folds

The dura mater is composed of two closely adherent layers (the outer endosteal layer and the inner meningeal layer). These layers separate only to enclose the dural venous sinuses, which drain venous blood from the brain, orbit, and adjacent skull bones.

The inner meningeal layer forms four distinct folds inside the cranial cavity:

A. Falx Cerebri

A sickle-shaped fold located in the midline between the two cerebral hemispheres.
- Anterior Attachment: Attached to the crista galli and frontal crest.
- Posterior Attachment: Blends with the upper surface of the tentorium cerebelli.
- Superior Attachment: Attached to the lips of the sagittal sulcus on the inner aspect of the skull cap.
- Inferior Border: Free, concave, and lower.
- Associated Sinuses:
- Superior sagittal sinus (runs in the upper attached border).
- Inferior sagittal sinus (runs in the free concave lower border).
- Straight sinus (runs along the line of junction with the tentorium cerebelli).

B. Tentorium Cerebelli

A tent-like fold that covers the cerebellum and supports the occipital lobes of the brain.
- Attached (Peripheral) Margin: Attached to the posterior clinoid process, the upper border of the petrous temporal bone, and the margins of the transverse sulcus on each side.
- Free Margin: Attached anteriorly to the anterior clinoid process, forming the tentorial notch around the midbrain.
- Nerve Relationships:
- The free margin crosses the attached margin at the petrous apex.
- The trochlear nerve (CN IV) pierces the dura at the exact point where these margins cross.
- The oculomotor nerve (CN III) pierces the dura just in front of this crossing point.
- Surface Attachments: Its upper surface gives attachment to the falx cerebri, while its lower surface gives attachment to the falx cerebelli.
- Associated Sinuses:
- Straight sinus (at the junction with the falx cerebri).
- Superior petrosal sinus (along the upper border of the petrous temporal bone).
- Transverse sinus (along the transverse sulcus).

C. Falx Cerebelli

A small, crescentic fold located between the two cerebellar hemispheres.
- Attachments: Attached to the internal occipital crest and the inferior surface of the tentorium cerebelli.
- Associated Sinuses: Occipital sinus and straight sinus.

D. Diaphragma Sellae

A small horizontal dural fold that covers the pituitary gland in the hypophyseal fossa. It is pierced centrally by the pituitary stalk.
- Attachments: Attached to both the anterior and posterior clinoid processes.
- Associated Sinuses:
- Anterior intercavernous sinus (in its anterior border).
- Posterior intercavernous sinus (in its posterior border).


2. Anatomy of the Dural Venous Sinuses

Dural venous sinuses are characterized by being valveless, lacking muscular tissue in their walls, and residing between the two dural layers. They are classified into single and paired groups.
